This membership form is a copy of the "paper" version, since we can not handle "internet" forms right now.

If you want to become a member or let us know (as member) changes in f.i. your address, please e-mail ALL the requested information to the STACK membership administrator or to the National Director.

You could also copy the form, and send it to either SIEC or the National director

For new members:

Fill in this form carefully and send it to the National Director in your country, to STACK international (SIEC) or to SNACK.

For those who already are a member and got this form from their ND, the information will be used to update existing lists

The top part of the form will be put on the mail you get from STACK, so make sure it is what you (and the post office !!) want. You can use three lines for the address, but always put "city/ postal code" and "country" on their proper lines.

The information you give will be in the STACK Membership list, if you have no objections, which will be available to STACK members

You don't have to fill in every line, and you can use "MISC" for things like a mobile phone number etc.
